Management usually begins with specific medications for neuropathic pain, but some patients require specialized procedures when the response is insufficient or side effects make it difficult to continue therapy.<\/p>\n

The choice does not depend solely on the intensity of the pain. The pattern of episodes, MRI results, response to previous treatments, age, and general health status must also be considered. A specialized evaluation<\/a> helps determine whether it is advisable to adjust medications, maintain clinical follow-up, or consider a neurosurgical alternative in Costa Rica.<\/p>\n

Treatment for Trigeminal Neuralgia<\/h2>\n

Trigeminal neuralgia causes a type of neurological facial pain that often presents as electric shocks, stabbing sensations, or sudden attacks on one side of the face. Because of its nerve origin, conventional analgesics used for muscle or inflammatory pain do not always provide adequate relief. Anticonvulsant medications are usually the first option to reduce the abnormal transmission of pain signals through the trigeminal nerve.<\/p>\n

Medications are usually the first step<\/h3>\n

Carbamazepine is one of the most frequently used drugs as initial treatment. Depending on the clinical characteristics and the patient’s tolerance, the specialist may consider other options, such as oxcarbazepine or other medications that act on nerve activity. The selection, dosage, and adjustments should always be under medical supervision.<\/p>\n

These treatments should not be started, changed, or discontinued without professional guidance. Some may cause drowsiness, dizziness, lack of coordination, nausea, or other side effects. In addition, certain patients require laboratory tests or regular check-ups to monitor their safety. Initial improvement does not mean that follow-up can be stopped, as the response may change over time.<\/p>\n

The goal is not only to reduce the number of attacks. The specialist<\/a> also assesses whether you can eat, speak, brush your teeth, sleep, and carry out your activities without constant fear of triggering pain. Keeping a record of the frequency, duration, intensity, and possible triggers of the attacks can provide useful information during consultations.<\/p>\n

Another alternative is stereotactic radiosurgery. Despite its name, it does not require a conventional incision. It uses highly focused radiation on a region of the trigeminal nerve to reduce pain signals. Relief may be gradual and not necessarily immediate. Radiosurgery is recognized as a non-invasive option for selected patients with trigeminal neuralgia.<\/p>\n

Procedures that act directly on nerve fibers may cause some degree of facial numbness. There is also the possibility that the pain may return and a new treatment may be needed. For this reason, the specialist should explain the expected benefits, limitations, and specific risks before making a decision.<\/p>\n

When to consider surgery for trigeminal neuralgia?<\/h3>\n

Surgery for trigeminal neuralgia may be considered when pain persists despite adequate medical treatment, medications stop working, or their side effects become difficult to tolerate. It can also be considered when MRI and clinical features suggest that a blood vessel is compressing the nerve near its entry into the brainstem.<\/p>\n

Microvascular decompression treats an identified compression<\/h2>\n

Microvascular decompression is a neurosurgical procedure aimed at separating the blood vessel that is pressing on the trigeminal nerve. Unlike techniques that selectively modify or injure nerve fibers, this surgery seeks to preserve the nerve and correct the vascular conflict responsible for the symptoms.<\/p>\n

The procedure requires general anesthesia and a small surgical opening behind the ear. The neurosurgeon<\/a> accesses the area where the nerve approaches the brainstem, identifies the vessel causing the compression, and places a separation to prevent direct contact. Its purpose is to review the diagnosis, interpret the images, assess the response to medications, and explain which option offers a reasonable balance between benefits and risks. The final decision should be informed and tailored to each person’s needs.<\/p>\n
